When a document is first saved the document title is set to the filename, no matter if the document has already been titled. PageStream should not overwrite the document title if it is not equal to an empty string (or 'untitled' which I guess is the actual default title name).

Document name now always set to document name (minus file extension), until you set the document name to something else in Document Setup.
